Help Guide

This help guide is provided to assist you with questions you may have when completing the online Personal Financial Statement.  If you need additional assistance, please contact your lender at

  Note: If you wish to print out the Help Guide, please change your printer settings to print horizontally.

 Type of Credit Requested:

  • Please indicate the type of credit you are applying for, secure or unsecure, by clicking in the radio button in front of the appropriate choice.
  • If you are completing this form for the purpose of updating your information only, please click in the radio button in front of "No Additional Credit is requested".
  • If you are completing the form for credit application purposes, please indicate whether you are requesting Individual or Joint credit by clicking in the radio button located in front of the appropriate choice.
  • Place your initials in the appropriate section of "Applicant Initial" and/or "Co-Applicant Initial" and click "Next".

Income Statement:

  • If you are the Applicant, please enter your first and last names and middle initial.
  • If you are the Co-Applicant, please enter your first and last names and middle initial.
  • Complete the Annual Income section for both the Applicant and Co-Applicant, if applicable.
  • Complete the Contingent Liabilities section.  (A Contingent Liability is a potential liability depending on a future event occurring or not occurring.  Examples are lawsuit expenses, or being a co-signer for a loan that you are now making  payments on due to default.)

Schedule 1 - Accounts Receivable:

  • Enter the name(s) of those debtors who currently owe you money.
  • Enter the name of the person these debts are owned to. (Applicant or Co-Applicant)
  • Enter the collateral that was provided by the debtor.
  • Enter how much money is paid to you within a time frame.
  • Enter the maturity date of when the debt is to be satisfied.
  • Enter the total amount of unpaid debt that is owed to you as Applicant or Co-Applicant.

Schedule 2 - Mortgage & Contract for Deed Owned:

  • Enter the name of the debtor,
  • Enter the type of the property the mortgage or contract for deed is on.
  • Enter whether it is a 1st or 2nd lien.
  • Enter how much money is paid and within what time frame.
  • Enter to Whom the money is owed.
  • Enter in the unpaid balance of the mortgage and/or contract for deed.

Schedule 3 - Securities Owned:

  • Enter the type of securities owned.
  • Enter the number of shares.
  • Enter the cost per share you paid.
  • Enter the name of the person the securities are registered in.
  • Enter whether the securities are listed or unlisted.
  • Enter the current market value of the security.

Schedule 4 - Life Insurance:

  • Enter the name of the Insured person.
  • Enter the name of the Insurance Company.
  • Enter the dollar amount of any loans against the policy.
  • Enter the name of the beneficiary.
  • Enter the face value of the policy.
  • Enter the cash value of the policy.

Schedule 5 - Real Estate:

  • Enter the address and type of property.
  • Enter the initial cost to purchase the property.
  • Enter the amount of monthly income received from the property.
  • Enter the name of the person in which the title is listed.
  • Enter the amount of insurance on the property.
  • Enter the market value of the property.
  • Indicate whether or not the property is a homestead by selecting yes or no. (A Homestead is defined as a personal home plus any additional buildings and adjoining land.  [Additional buildings could be garages, quonsets, outbuildings, etc.]) 

Schedule 6 - Mortgages or Liens on Real Estate:

  • Enter to whom the mortgage or lien is payable to.
  • Enter the interest rate of the mortgage or lien.
  • Enter the maturity date of the obligation.
  • Enter how much and how often the payment amount is paid.
  • Enter the total amount of the unpaid balance.

Schedule 7 - Notes Payable to This Bank:

  • Enter the purpose of the loan.
  • Enter the interest rate of the loan.
  • Enter the maturity date of the loan.
  • Enter how much and how often the payment amount is paid.
  • Enter the total amount of the unpaid balance.

Schedule 8 - Notes Payable To Others:

  • Enter the name and address of the person the loan is payable to.
  • Enter the interest rate of the loan.
  • Enter the description of the collateral securing the loan.
  • Enter how much and how often the payment amount is paid.
  • Enter the total amount of the unpaid balance.

Schedule 9 - Business Ventures:

  • Enter the name of the business you are a principal owner of or partner in.
  • Enter the address of the business.
  • Enter your position or title within the business.
  • Enter the line or type of business.
  • Enter the number of years you have been involved in the business.
  • Enter the % of ownership you hold within the business.
  • Enter the present net value of your investment.

Assets and Liabilities:

  • Review the collected list of asset and liability values for accuracy.
  • Complete the Yes and No questions for the Applicant and Co-Applicant (if applicable) by clicking in the radio box in front of the appropriate answer.
  • Click Next

Add Tax Returns:

If you have tax returns you would like to provide, please save them in pdf format and perform the following:

  • Enter the year of the tax returns you are attaching in the text box.
  • Browse for your file and click Add.  (Complete these two steps to add additional tax returns.)
  • Once completed attaching tax returns, click Next.

If you do not have tax returns you would like to provide, click the Next button.

Notice of Negative Information:

Read the "Notice of "Negative Information" and finish completing the form by clicking where indicated to state you have read and agree to the disclaimer above.  This step also needs to be completed by the Co-Applicant if applicable.
  • Please indicate whether or not you have a current loan Officer.
  • If applicable, please enter the name of your loan Officer.
  • Submit the form.
